龋齿几乎存在于全球每个人的口腔中。传统的治疗或预防策略通常基于清除所有口腔微生物,而不仅仅是致龋菌。生物制剂方法则提供了潜在的生物干预新策略,旨在通过将有益的生物成分引入口腔,恢复天然口腔微生物群并维持平衡。
随着人体干预研究的不断积累,未来该领域可能会出现更多健康声称。龋齿的风险因素和原因包括口腔生物膜中高数量的变形链球菌、糖类产酸、牙菌斑形成及其厚度,最终结果影响DMFT指数(龋失补指数)。因此,针对生物膜进行干预应是降低龋齿风险的主要策略。
益生菌对口腔健康的益处
益生菌是第一个被明确定义的生物制剂(根据ISAPP的定义),最早被应用于早期龋齿研究,其中包括芬兰的Jukka H. Meurman教授——他是牙科生物膜、口腔微生物群以及特定益生菌干预研究的先驱。在他的工作之后,许多后续研究成功测试了各种生物成分在牙科中的应用。在目标方面,益生菌已被评估其对口腔微生物群组成的影响、对口腔黏膜的黏附能力、对黏附位点的竞争以及对已附着潜在病原体的置换作用。除了预防龋齿,益生菌在维护牙周健康方面也显示出效果,例如使用含有罗伊氏乳杆菌的含片在长期海上航行的海军水手中维护牙周健康。这些益生菌研究还揭示了可能与其他生物制剂共享的许多机制。
木糖醇及其他益生元
欧洲食品安全局(EFSA)评估的益生元领域最早获批的健康声称之一,就是木糖醇作为口香糖成分每日食用可降低龋齿风险。优雅的人体研究证实了风险因素的变化,包括变形链球菌水平的降低、口腔pH值的平衡以及DMFT指数的有利变化。尽管有报道称微生物能适应不同底物,但变形链球菌无法将木糖醇作为碳源进行适应,从而阻止了酸的形成。随后,其他多元醇也显示出类似特性,并且关于益生元支持口腔和鼻咽部微生物群作用的信息现已更加丰富。
结合成分创造合生元
几种符合ISAPP合生元定义的组合已被使用,并取得了有前景的机制性结果,包括微生物群调节和防止pH降低。碳水化合物(如低聚半乳糖)与益生菌的组合已显示出有助于制定人体干预研究产品的特性。
非活菌用于口腔健康益处
由ISAPP定义的后生元在科学和商业上都引起了浓厚兴趣,部分原因在于其产品配方的工艺特性。后生元在口腔健康中的应用侧重于在不同产品形式(包括含片、牙膏和乳制品)中具有良好的酸碱稳定性和热稳定性。目前许多研究正在进行中,结果预计很快公布。
