必须条件（确诊依据）：
组织病理学特征：
皮肤活检显示典型肉芽肿结构，核心包含巨噬细胞及其衍生的上皮样细胞
存在多核巨细胞（异物型或Langerhans型）
周围淋巴细胞浸润（以CD4+ T细胞为主）
支持条件（临床依据）：
典型皮损表现：
持续≥4周的皮肤丘疹/结节（直径>5mm）
边界清晰的斑块伴红褐色色素沉着
排除感染性肉芽肿（结核、麻风等）
特殊分布模式：
面部/四肢伸侧对称性分布（阳性预测值>80%）
排除标准：
无系统性肉芽肿疾病证据（胸部CT无肺门淋巴结肿大）
血清ACE水平<40 U/L（排除结节病）
