恶性间叶性肿瘤（Malignant Mesenchymal Tumor），又称恶性间叶瘤，是一种由两种或以上不同类型间叶组织（包括肌肉、脂肪、纤维组织、骨、软骨和血管等）共同构成的罕见恶性肿瘤。其不同于单一组织来源的肉瘤（如骨肉瘤、脂肪肉瘤），特征在于同一肿瘤内存在多种独立分化的恶性间叶成分（如同时含有横纹肌肉瘤和骨肉瘤成分）。此类肿瘤具有高度侵袭性和异质性，可发生于各年龄段，预后与肿瘤分化程度、成分类型及解剖位置密切相关。临床表现以快速增大的无痛性肿块为主，可伴随局部压迫症状或器官功能障碍。

二、医学定义

病理学上，恶性间叶性肿瘤需满足两个核心特征：①肿瘤内至少包含两种明确分化的间叶源性恶性成分（如平滑肌肉瘤与软骨肉瘤并存）；②各成分间不存在组织学过渡，需排除去分化肉瘤或化生性癌。其发生机制涉及多能间叶干细胞的恶性转化及分化路径异常，常伴随TP53、RB1等抑癌基因失活及表观遗传学改变。诊断依赖组织病理学检查及免疫组化标记（如MyoD1、SMA、S100等），需与癌肉瘤、混合型肉瘤等鉴别。

三、病因：病原学机制

遗传易感性：
- 部分病例与遗传性肿瘤综合征相关，如Li-Fraumeni综合征（TP53突变）、神经纤维瘤病I型（NF1突变）等。
体细胞基因突变：
- 染色体复杂重排（如12q13-15扩增）、MDM2基因过表达及PI3K/AKT/mTOR通路异常激活常见。
放射线暴露：
- 电离辐射（尤其是既往放疗史）是明确风险因素，潜伏期可达10年以上。
化学致癌物：
- 长期接触氯乙烯、砷剂等与血管肉瘤发生相关，但与其他亚型关联证据有限。
