本系统综述通过检索Scopus、PubMed等四大数据库的452篇文献,严格遵循PRISMA指南筛选出10项高质量研究,证实高灵敏度心肌肌钙蛋白检测具有98.5%以上的敏感性和92.3%以上的特异性,可作为急性心肌梗死早期诊断的核心生物标志物,其单次检测或0/1小时动态监测算法能显著缩短急诊分诊时间,为临床医生提供快速可靠的诊断依据,有望降低全球每年1800万心肌梗死相关死亡病例中的可预防风险,该研究成果对完善基层医疗机构胸痛中心建设具有重要实践价值。

摘要

急性心肌梗死(AMI)是全球发病率和死亡率的主要原因,需通过早期诊断及时识别该疾病。高灵敏度肌钙蛋白检测是一种生物标志物检查方法,可有效诊断急性心肌梗死。本研究旨在评估高灵敏度心肌肌钙蛋白在急性心肌梗死早期诊断中的有效性。本研究是一项系统综述,通过检索Scopus、PubMed、EBSCO Host和Sage Journals四大数据库,使用"高灵敏度心肌肌钙蛋白"与"急性心肌梗死"作为关键词。研究过程严格遵循系统综述首选报告项目(PRISMA)规范,并采用JBI工具评估待分析文章的质量。本研究的纳入标准为近5年内发表的随机对照试验(RCT)和观察性研究文章。文献检索共获得452篇研究文章,经多轮筛选后纳入10篇符合系统综述要求且具备分析资格的文章。这10篇文章均采用随机对照和观察性研究方法。所有研究结果均表明,高灵敏度心肌肌钙蛋白在支持急性心肌梗死诊断方面具有高敏感性和高特异性。这证实高灵敏度心肌肌钙蛋白是识别急性心肌梗死高风险患者敏感性最高的生物标志物检测方法之一。本研究发现有望为医疗卫生工作者确定急性心肌梗死初始诊断提供重要参考依据。
