手术后认知功能障碍:一个新兴问题

摘要
术后认知功能障碍(POCD)是一个重大问题,会导致生活质量下降,并增加发病率和死亡率,尤其在老年患者中。接受手术的患者年龄增长使这一问题日益凸显,多项研究评估了其发病机制并实施了靶向治疗。本综述分析了POCD、谵妄和轻度认知障碍(MCI)的定义、风险因素和病理生理学,并探讨了麻醉技术的影响。与术后谵妄不同,POCD更难诊断,因为需要在术前和术后(甚至间隔一段时间)进行多项测试。此外,通过脑电双频指数(BIS)或脑电图(EEG)监测适当的麻醉深度可降低认知功能障碍的发生率。目前发病机制尚不完全清楚,尽管神经炎症起核心作用,而麻醉技术的重要性尚未确定。

引言
麻醉和手术技术的不断进步使得手术操作成为可能,也适用于老年患者。据估计,所有手术操作中有30%是在老年人身上进行的。然而,尽管发病率和死亡率已有所降低,但术后并发症在这一人群中仍然常见。事实上,改善术前功能以及改良麻醉和手术技术已导致心血管、代谢、肺部和呼吸系统并发症的减少。但一个令人担忧且日益严重的问题已被识别:即术后神经功能改变,也称为术后认知功能障碍(POCD),它难以预防,治疗起来更加困难[1-3]。
术后神经生理功能障碍是一个风险因素,直到几年前才被考虑,即从越来越多老年患者接受麻醉和手术时起。这个问题并不新鲜,家属凭直觉就能察觉。事实上,经常听到亲属说“我丈夫(或妻子)在做完手术后就不一样了”。因此,这已被认为是一个重要的术后问题,不仅从围手术期角度,而且在长期内也是如此,因为认知障碍可能进展为痴呆[4]。然而,为了理解诊断差异及其对预后和治疗的影响,引入了围手术期认知功能障碍的临床定义。
具体来说,本综述将探讨谵妄、轻度认知障碍(MCI)和术后认知功能障碍(POCD)的定义,但重点将放在POCD上。
谵妄
术后谵妄是一种常见的不良事件,约20%至60%的老年住院患者出现。它发生在手术后不久,但在接受通气治疗的重症监护病房(ICU)患者中发生频率更高,并且仍然是一个重要的临床问题和真正的医疗急症[5,6]。谵妄在术后出现得相当早,最常见于术后第一天,通常在手术操作后24至72小时内,其特征是注意力和思维的突然改变,伴有精神状态波动。其持续时间从几天到几周不等,并与认知能力下降以及发病率和死亡率增加相关[7]。
必须将其与麻醉苏醒期谵妄区分开来,后者是短暂的,发生在年轻人身上[8]。在许多病例中,症状在几天或几周后消失,但也可能持续存在并导致进行性认知能力下降,最终发展为痴呆。在SAGES(择期手术后成功老龄化)研究中,Inouye等人报告了有认知能力下降和无认知能力下降患者的双相行为:一个月后出现重大认知下降,两个月后观察到功能的可比性恢复,但最重要的是,在麻醉或手术后36个月,患者出现了加速的认知下降[9]。Gross等人证实,谵妄后的POCD会加速阿尔茨海默病患者的认知下降[10]。
谵妄可出现三种形式:即活动过度型、活动减退型和混合型。活动过度型约占四分之一,容易识别,而活动减退型表现为疲劳和活动减少,常被误认为是嗜睡。在ICU中,可使用谵妄评估方法(CAM)诊断谵妄,而护理谵妄筛查量表在恢复室中有效。两种方法都具有较高的灵敏度和特异度[11,12]。尽管已确定多种病理因素是术后谵妄的风险因素,但生理因素也起着重要作用。接受髋部手术区域麻醉的患者有意外的主要风险因素,而近期使用低剂量右美托咪定已显著减少这些并发症,尽管没有令人信服的证据表明药物治疗或预防有效[13-15]。谵妄在常规临床实践中仍未被充分识别,应作为真正的急症处理。
轻度认知障碍
轻度认知障碍(MCI)是一种临床实体,其特征是记忆力损伤比年龄和教育水平预期的更严重,代表了正常年龄认知改变与阿尔茨海默病早期状态之间的过渡阶段[16,17]。MCI在术后可演变为POCD,因此术前通过神经心理学测试进行仔细评估对于检测认知能力下降是必不可少的。Randolph等人引入了一套有用的敏感测试集用于MCI诊断:可重复的神经心理状态评估成套测验(RBANS)[18]。
RBANS是一套简短而全面的神经心理学成套测验,由12个单独施测的分测验组成,用于评估五个不同的认知域:即时记忆、视空间/结构能力、语言、注意力和延迟记忆。RBANS可在30分钟内完成施测,获得五个域各自的指数分数。该测试可用于评估20至80岁患有器质性、神经心理学和精神问题的患者。
Kotani等人验证了RBANS在评估花生四烯酸(ARA)和二十二碳六烯酸(DHA)给药前后神经心理学状态的有效性。该研究表明,膳食补充剂改善了有器质性脑损伤或衰老患者的MCI[19]。
术前认知障碍的存在是术后认知功能障碍发生的重要风险因素。这个问题对麻醉医师尤其重要。Trowbridge等人发现,在85岁以上的泌尿妇科患者中,MCI患病率很高(高达30%)[20]。其他作者估计,在择期手术的老年手术人群中患病率更高[21]。Silbert强调了手术操作和麻醉可能影响或损害有MCI患者的神经功能这一概念。在该研究中,所有患者通过七项神经学测试评估是否存在MCI。有MCI的受试者术后功能障碍的发生率增加,并且在术后3至12个月之间出现持续的认知下降[22]。这一结论得到Liu等人的证实,他们表明认知下降的发生率更高,这在接受七氟烷暴露的受试者中最为显著,与接受静脉或硬膜外麻醉的受试者相比[23]。
认知功能障碍
认知功能是感知、记忆、注意力和信息处理的心理过程。大脑处理外部信息,组织这些输入以获得时间和空间定向,发展语言、学习、集中注意力和执行功能以进行日常活动。认知功能障碍被定义为这些功能中的一个或多个发生改变:注意力、工作记忆、知觉和运动能力。然而,认知功能障碍是一种隐匿的认知下降,通常难以诊断,只能通过一系列神经心理学测试来揭示。ISPOCD1和ISPOCD2(国际术后认知功能障碍研究)中使用的一系列神经心理学测试适用于且敏感于检测术后期间的POCD[24]。这些测试显然是用英语开发的,因此必须翻译和改编以便在其他国家使用。
神经心理学测试应在术前和术后不同时间进行,因为认知恶化在手术后不久出现,并在大多数患者中于数天或数周后减轻,但可能持续数周、数月和数年[25]。诊断非常困难,因为神经心理学评估应在术前和术后进行,但这些测试的施测需要时间,最好由专家进行。认知功能的评估基于术前和术后测试的结果。精确的测试施测时间尚不清楚。有时,当重复进行神经心理学测试时可能出现的正确“练习效应”被用作对照组[26,27]。
因此,确切的发生率未知,并且很可能被低估,直到引入简单而敏感的测试。大多数POCD研究存在若干方法学问题:即POCD定义不标准化,测试不统一,并且测试在术后不同时间进行[28]。
例如,简易精神状态检查(MMSE)评估推理、时空定向和记忆,但不能用于研究POCD。ISPOCD工作组进行了多项研究并提出了若干建议。神经心理学测试经过讨论,并建立了共识声明[29,30]。POCD定义为超过2.0个测试的评分。报告的POCD发生率在术后7天为7%至71%,在术后两或三个月为6%至56%。
特别是,麻醉药的效果在人类中难以评估,因为存在预先存在的退行性神经过程、多器官合并症和手术应激。
病理生理学
POD和POCD的病理生理学尚不十分清楚。尽管近年来对POCD进行了广泛研究,但病理生理机制仍不清楚,有待确定。然而,在发病机制方面已取得进展。神经炎症似乎在神经认知能力下降的发展中起突出作用,正如在神经退行性疾病(如帕金森病和阿尔茨海默病)以及与年龄相关的认知下降中所证明的那样。麻醉和手术可能协同作用。促炎细胞因子与谵妄之间的相关性似乎已得到证实。由于促炎细胞因子导致胆碱能活性降低或多巴胺增加,伴随小胶质细胞激活诱导神经损伤和术后谵妄的发生。白细胞介素-6、白细胞介素1β、干扰素、皮质醇肿瘤坏死因子-α(TNF-α)的增加已被证明会降低胆碱能活性[31,32]。术后POCD中的许多生物标志物与术后炎症相关,并参与神经退行性疾病和POCD的进展。促炎细胞因子的增加似乎代表了炎症与应对其他免疫细胞(如肥大细胞)信号的小胶质细胞启动之间的联系,这些细胞可能被手术创伤外周激活[33]。
小胶质细胞是免疫细胞,激活时通过释放引起神经元损伤的炎症介质在发展和维持炎症中起重要作用,导致细胞凋亡和认知能力下降,尤其在老年患者中[34]。其他炎症介质与POCD相关:蛋白S100A8、促炎和调节性CD4+ T细胞的修饰降低了内毒素免疫力[35,36]。淀粉样肽沉积增加和星形胶质细胞增生也被涉及短期POCD,显示了与阿尔茨海默病相似的机制[37]。神经炎症可能涉及其中,Wang等人已在实验中强调。给予米诺环素(一种抑制促炎细胞因子产生并促进小胶质细胞释放抗炎细胞因子的抗生素)可诱导认知障碍的减轻[38]。给予N-乙酰半胱氨酸可能通过抗氧化和抗炎作用来损害POCD,该作用通过产生谷胱甘肽(一种强效的抗氧化和抗炎剂)抑制神经炎症细胞的激活[39]。Skvarc等人开展了一项有前景的研究,评估在手术前一天和手术后四天口服补充N-乙酰半胱氨酸的效果。该药物抑制或减少了认知障碍[40]。
这些中枢神经系统和外周炎症增加的生物标志物与动物和人类实验研究中的脑灰质改变相关。麻醉的目标是中枢神经系统,并且术中脑改变可能持续到术后,即使麻醉药的效果消失。
特别是,已证明在接受手术的患者中,参与认知功能的区域(海马和大脑皮层)出现脑灰质丢失。术后较低的海马体积或海马体积减少已被认为是围手术期POCD的风险因素[41]。使用脑MRI,Sato等人评估了32名接受乳腺癌手术患者的灰质体积。在术后即刻,灰质体积减少,尤其是丘脑,与POCD下降相关,特别是在注意力方面[42]。
然而,在老鼠实验研究中,Valentim等人报告了不同浓度异氟烷的不同效果。较低浓度的麻醉药似乎比较高浓度更危险。事实上,在使用较低浓度异氟烷的情况下,已报告海马细胞凋亡增多和空间学习障碍[43-45]。

麻醉类型能否影响POCD在很大程度上是一个争论的问题。
挥发性麻醉药似乎是独立的危险因素,至少在一些动物研究中是这样。老年大鼠暴露于挥发性麻醉药异氟烷可通过激活老年大鼠海马中NF-κB信号通路的IL-1β而诱发认知缺陷[46]。然而,其他作者未确认幼年大鼠在暴露于七氟烷和地氟烷后空间记忆的下降[47]。挥发性麻醉药对神经元细胞的直接影响已在诱导神经功能障碍的实验研究中得到证实,这似乎与不同挥发性麻醉药之间不可比。异氟烷(一种近期较少使用的麻醉药)已被证明比地氟烷和七氟烷更具细胞毒性和促凋亡性,可被视为一个促成原因[48]。相反,丙泊酚似乎通过减少诱导细胞毒性的酶促级联反应而具有保护作用。然而,丙泊酚与吸入麻醉药之间的结果难以捉摸。Egawa等人未发现丙泊酚和七氟烷在肺部手术患者中对POCD和脑氧合有显著差异,其他作者在小型手术后也证实了相同结果,强调炎症并非唯一原因[49,50]。Qiao等人在一组接受食管癌切除术的患者中发现,使用丙泊酚麻醉或接受皮质类固醇治疗的受试者POCD显著减少,从而强调了炎症的相关作用。
有两项研究评估了阿片类药物对POCD应激的影响。阿片类药物的效果仅得到了边缘性评估,只有少数研究发表。一项对622名接受腹部大手术患者的研究评估了芬太尼单次注射或输注瑞芬太尼(0.15至0.25 mg/kg/min)是否能改变促炎细胞因子和术后时期。用BIS监测麻醉深度。该研究的意义在于POCD发生率较低,即19.2%,两组之间无显著差异,尽管芬太尼组的促炎性IL-6在第七天之前较高。吗啡和其他阿片类药物抑制前细胞因子可减少炎症表达,但似乎对POCD发生率无影响[51]。
其他研究也获得了相同结果。高低剂量芬太尼的比较显示POCD发生率更高,但仅在第一周内,在三个月和十二个月后则无差异;而Rasmusses等人未证明瑞芬太尼和舒芬太尼组在POCD方面的差异,尽管通气时间和低SvO₂与术后认知下降相关[52,53]。
选择局部区域麻醉似乎是一种最佳技术,以减少伤害性传入、术后疼痛和炎症。已对腰麻和硬膜外麻醉的效果进行了研究,尤其是在骨科手术中,这种手术在术前有认知下降的老年患者中特别常见。已发表了多篇文章和综述,但在此案例中也未给出明确答案。尽管进行了大量前瞻性和随机研究,但仍无法得出结论。原因是研究异质性大:不同类型的手术、研究POCD的方法学、评估时间窗和术后镇痛。特别是,大多数研究是在老年患者、骨科手术和不同类型的术后镇痛(尤其是吗啡)中进行的。Rasmussen等人进行的一项非常好的研究未发现全身麻醉、腰麻或硬膜外麻醉之间POCD发生率的差异,但死亡率不同,正如可预测的那样[54]。Davies等人在一项系统评价中比较了全身麻醉与局部区域麻醉,在16项随机对照研究中,只有4项研究报告了全身麻醉与局部区域麻醉之间的差异:Hole的早期研究报告了局部区域组更好的结果,与Mandal的研究一致,后者报告了MMSE、言语流畅性以及即时和延迟记忆测试方面的显著差异。这些研究的局限性在于患者数量、不同的麻醉技术(硬膜外、腰麻)、研究时机和所使用的测试(与特定的神经生理学测试相关)[55-57]。
同年,Scott等人在一项荟萃分析中评估了接受全关节置换术患者的POCD发生率,该荟萃分析调查了17项研究中的1,089名患者,但只有两项研究报告了对照组。尽管多年来进行的研究在认知测试施测和麻醉类型方面存在许多差异,但手术似乎对认知功能影响很小或没有影响,尤其是在出院后3至6个月。仅在术后早期报告了即时语言回忆的减少[58]。
风险因素
如果麻醉不是风险因素,那么促成因素是什么?风险因素可能与患者相关。主要原因包括老年患者的虚弱、术前认知障碍、血管性痴呆、阿尔茨海默病、酒精或药物滥用以及文化水平。对POCD的易感性与特定前额叶和颞叶区域体积的丧失、尤其是突触活性降低和脑神经递质减少有关[59,60]。年轻脑与衰老脑之间存在不同行为,这在麻醉患者中临床上很明显:MAC或丙泊酚和阿片类药物在手术中的浓度效应随年龄增长而降低。何时以及针对什么情况需要对有风险或患有POCD的患者进行麻醉剂量的滴定:通过BIS或EEG监测适当的麻醉深度可使受试者处于较低的麻醉浓度,从而减少谵妄发生的可能性和POCD的发生率[61,62]。另一个风险因素是术中和术后低氧血症的发生以及未控制的术后疼痛[63,64]。众所周知,严重缺氧会损害大脑,长期低氧张力可诱发脑损伤。然而,除了已提到的因素外,大量围手术期因素也与POCD相关,即禁食、液体平衡、睡眠剥夺、疼痛和心理特征。
结论
自1955年Bedford引入POCD概念以来,已开展了多项研究,将其确认为一个重大问题:通过损害日常活动降低生活质量,增加1至5年内的发病率和死亡率,尤其是在特定手术后:心脏手术、骨科手术和急诊手术[65]。由于接受麻醉和手术的患者中有很高比例是老年人或高龄老人,因此不仅要注意术后心血管和肺部并发症,还要注意术后认知功能障碍、谵妄和POCD。这种情况的真实影响和患病率并不像POCD那样为人所知,只能通过神经心理学测试检测。只有谵妄和严重的术后缺陷才能被检测到。
然而,尽管一些研究已开始阐明病理生理学的一些黑点,但在预防和治疗方面取得的进展甚微。毫无疑问,神经炎症在其发病中起关键作用,但麻醉和手术的具体作用尚不明确。特定的麻醉药被认为有利于术后脑改变,就像某种类型的手术一样,但结果尚无定论,尤其是在3至12个月后评估POCD时[66]。临床术中和术后风险因素已得到阐明,但只有针对预防或治疗的特定研究。加强术中监测,特别注意脑氧合和麻醉深度,目前尚无特定的神经保护技术或药物,零星的发表物概述了某些药物的保护作用。特别是,动物研究表明利多卡因可抑制细胞因子的产生,在人体中,右美托咪定也显示出这种作用,能够减少炎症反应和早期POCD[67-69]。
