引言

除了炎症,较高水平的血清脑源性神经营养因子(BDNF)——解释为指示更大的神经可塑性——与更好的认知表现相关[32],而较高的瘦素与基线SDMT较差相关,有随后恶化的趋势[33]

扩展到脑脊液(CSF),除了κ自由轻链比率指示的鞘内B细胞激活[34,35]外,还有几种额外的标志物与认知相关。与血清发现相呼应,较高的CSF NfL浓度与较差的认知表现显著相关[36],CSF BDNF也报告了类似的相关性[32]。最近,较高的CSF几丁质酶-3样蛋白1(CHI3L1/YKL-40)——一种主要由活化的星形胶质细胞和小胶质细胞分泌的几丁质酶样糖蛋白——与认知障碍相关,特别是在进展型MS中[37]。此外,CSF parvalbumin,一种由快速放电GABA能中间神经元表达的钙结合蛋白,被发现与诊断时认知障碍的严重程度相关[38],并能够预测四年的认知下降[39]

在过去的一年中,脑连接作为认知功能的基底越来越受到关注。一项大型MAGNIMS分析整合了结构、扩散和静息态fMRI,显示MS患者脑的多路核心-外围组织的破坏更大,并与残疾和SDMT损伤相关,提供了超越传统测量的解释价值[48]。病变图、白质完整性、灰质体积和功能连接的联合独立成分分析(ICA)融合,识别出共同波动的结构-功能模式(特别是涉及丘脑辐射、胼胝体和感觉运动/丘脑区域),这些模式将MS与对照组区分开来,并比传统MRI更好地解释认知功能障碍[49]。在另一项研究中,结合灰质和白质特征的联合ICA与所有认知领域相关,并在执行功能和视觉记忆方面优于单组织ICA,强调了通过应用这种技术确定特定领域机制基底的机会[50]。最后,仅使用常规MRI,个体水平的结构断开和形态相似性网络对MS相关损伤敏感,解释了身体和认知残疾(包括SDMT)——关键的是——独立于病变负担和萎缩预测长期确认的残疾进展,将基于网络的指标定位为实用的预后和监测候选者[51]

在检查认知功能的长期预测因素时,灰质病理仍然是主导因素。纵向皮质病理已出现为特定领域下降的驱动因素:在一项为期10年的队列研究中,较高的皮质病变负荷和新皮质病变的发展与注意力、言语记忆和执行功能的更急剧恶化相关,独立于整体萎缩[52]。这表明皮质脱髓鞘以功能选择性方式塑造认知轨迹。与此一致,另一项20年随访研究表明,语言区灰质萎缩更大——包括楔前叶、岛叶、海马旁回和扣带回皮层——与二十年后更严重的认知障碍相关[53]

认知表型:我们的现状?

在过去十年中,实证工作已从二元的"受损/保存"标签转向描绘MS中可重复的认知表型,产生了缺陷模式及其生物学相关因素的更细致的图景。使用保守的、基于规则的方法,Hancock及其同事应用Jak/Bondi标准跨五个领域——注意力/处理速度、执行功能、语言、视空间能力和记忆——并且,在常用的临界值(1.0和1.5标准差低于规范均值)下,根据受损领域得分的数量(0-1"完整",2-3"单领域",4-5"双领域",≥6"多领域")对患者进行分组;注意力/处理速度被确定为最常受影响的领域[56]。扩展到纯粹的认知指标之外,Podda等人采用潜在类别分析结合情绪测量(焦虑和抑郁),确定了四种临床不同的特征:孤立的记忆困难;伴有情绪障碍的轻微记忆-语言缺陷;涵盖记忆、语言和注意力的中度损伤;以及一种严重的、广泛的模式,包括记忆、语言、注意力、信息处理和执行控制[57]

使用完全数据驱动的框架,我们推导出五种表型——认知保存;轻度言语记忆/语义流畅性;轻度多领域;严重的执行-注意力;和严重的多领域——并通过每个类别的独特MRI特征展示了聚合效度,支持表型边界的生物学合理性[58]。在一项独立的数据驱动研究中,Van Dam及其同事同样确定了可区分的表型,特别是以视空间记忆缺陷为特征的一种[59],而将相同方法应用于BICAMS结合Beck抑郁量表-II和改良疲劳影响量表,揭示了一个具有孤立神经行为障碍的亚组,强调了情绪和疲劳作为MS认知特征组成部分的相关性[60]

在所有研究中,MS队列与健康对照之间的组级认知差异一致但通常较为温和,常常达不到传统的1.0-1.5标准差阈值[61];因此,基于阈值的分类往往未能检测到细微但临床意义的改变,并在"受损"类别内合并异质性患者[62]。相应地,SDMT——虽然在二分规则下最常被标记的测量——被证明是多因素的,反映了处理速度、工作记忆、视感知处理、语言、感觉-运动功能和执行控制的贡献;在数据驱动的解决方案中,SDMT损伤聚集在更严重的表型中,而不是作为孤立处理速度损失的单一标志。综合起来,这些结果表明MS中的认知障碍是有结构的,具有重复出现的、生物学锚定的表型,比二元标签更好地捕捉缺陷的广度和模式。它们还表明,整合情感和疲劳测量可以改进亚组检测,而多变量、数据驱动的模型相对于逐项测试阈值提供了优越的结构效度。虽然基于规则的系统仍简单适用于个体,但累积的证据表明,由影像学相关支持的表型水平分类提供了对认知异质性更忠实的描述,并为有针对性的康复和未来基于机制的治疗提供了更强大的平台。

认知衰退的治疗

与认知功能障碍治疗机会相关的主要发现之一来自OPERA I/II的事后分析,报告显示在96周内,ocrelizumab比干扰素β-1a带来更大的SDMT增益,强化了对处理速度的治疗效果,超越了复发控制;互补评估表明nataziumab同样可能在减少病变累积和萎缩的同时保护认知[63]。认知康复产生了可测量但往往短暂的益处:Guttmann NeuroPersonalTrainer的多中心随机试验改善了言语记忆并增加了MRI衍生的网络连接[64],而进展型MS中的CogEx项目展示了区域灰质和网络变化,对主要处理速度结果有适度的转移[65]。非侵入性神经调节作为有价值的辅助手段出现:系统评价表明,经颅直流电刺激(tDCS)与认知康复配对可产生小到中等程度的改善(特别是处理速度/工作记忆),并且越来越多地可行在家中实施[66]。总体而言,虽然没有专门针对认知的疗法获得批准,但将高效疾病修正治疗与认知康复和神经调节相结合可能是最有价值的方法。

结论

综合起来,认知功能应被视为MS活动和进展的核心维度。常规、连续评估——至少包括SDMT,必要时辅以简表——可以检测急性复发相关的下降,包括偶尔的孤立认知复发,以及表征PIRA的缓慢下降。解释应以汇聚的生物学为基础。在液体方面,sNfL主要与复发性疾病中的认知相关,而新兴的CSF标志物(例如,CHI3L1、parvalbumin)和突触蛋白(SNAP-25、neurogranin、β-synuclein)加强了与神经元网络完整性的联系。在MRI上,灰质病理——皮质病变和区域萎缩,尤其是丘脑和语言区域——仍然是最有信息量的长期预测因子,而新型读数(深核中的QSM铁、丘脑放射组学、结构-功能网络指标,以及可能的脉络丛/类淋巴指标)增加了机制和预后特异性。临床上,高效DMT显示出保护处理速度的最令人信服的信号,但没有药物被批准用于认知;结构化的认知康复,可能通过tDCS增强,提供了额外的尽管往往是短暂的改善。展望未来,测试面板、阈值和练习效应处理的共识,表型感知的多模式监测,以及与以患者为中心的结果的纵向验证,对于将认知表型纳入常规护理并将干预措施针对最可能受益的人群至关重要。
